I have noticed that when I have used ATMs in the past in MX, there is a fee deducted from my cash withdrawal which would not qualify for refund because it is not separate.

That sounds like the Dynamic Currency Conversion charge.

I don't think so. It is the 3rd screen in your sequence that I am asking about, not the conversion rate. My experience is the same as yours in that the transaction ends if I do not accept this surcharge fee.
