Got some rinse water in first stage

Cap is standard issue SP. Reg was purchased a year ago. Cap does not have insert and appears to be in perfect condition.

When I do my soak, I now plan to ensure that my first stage does not get too deep in tub. Perhaps this caused the leak. Again, this is unprecedented. Thank you.
 
You might want to try my solution: I use a #4 lab grade rubber flask stopper. It's tall enough that when you tuck it in the yoke, you can really screw it down tight onto the yoke retainer. Much tighter seal than any cap.
Just don't get cheap Chinese stoppers. Their surfaces are irregular and don't seal reliably.
 
This old style “dust” cap is the best for a yoke reg, seals 100% and exposes all wet areas for soaking. I believe APEKS uses the same style.
