Update: I took the valve apart (mostly) and cleaned it up the best I could. Put it back together with existing parts because they all looked OK. Happy to report that the tank is no longer leaking. I'm really not sure if it was the O Ring in the neck or burst disk or anything else. But now I know what the inside of my valves look like.
I have a sneaking suspicion that it's the small paint chips around the neck on tank where valve seats that caused the problem. I made sure to tighten in down harder than I normally do without over torquing it. Two days and no leaks so far.
