Has anyone ever heard about a yoke type regulator not fitting onto a tank valve? Reg in question is older sherwood 3500, tank brand is unknown. Is there some type of standard valve out there that I am not familiar with? Scott

Yep. A lot of the older regulator yokes are too narrow to use on the wider, modern DIN/yoke convertible valves with the inserts. You need to find yoke only valves (without inserts) to use the older regs.

They still make yoke only valves that work with older regs. When buying a valve, simply specify that is what you need. I bought a new Catalina AL 80 a year and a half ago and specified that I needed a yoke only valve for my Conshelf regs.
