I looked it up in the Deloach book and it might be a Townsend Angelfish. In the book it states that sometimes Queen and Blue Angelfishes occasionally interbreed, resulting in offspring that share some distinctive markings from both species. For example: may have blue crown without central specks and/or half yellow tail,etc.
 
Some sort of Hybrid Queen Angel I suspect or maybe some sort of Deformity?

Bubble shot is amazing, as is the close-up of the damsel face and the flamingo tongue. What is the ID on the horned angelfish? I missed seeing him completely.
