I seem to be in the minority who still use paper logs, what can I say there's just something I like about being able to physically flip through my dive history. I also like to be able to write my thoughts or draw pictures immediately after the dives. I originally made my own template using a word processing program but found this template here on SB last year:

I've printed them on both sides of a standard 8 1/2 x 11 piece of paper which gives me 4 dives per piece of paper once they're folded in half, 3 hole punched and put in the day planner I use as my log book.
I'm not sure if I uploaded the template correctly so if it doesn't work, let me know and I'll figure out how to point you to it.
