There two problems with questionaires that come to mind.cancun mark:Yes they do, they send student questionares to about one in three students, or more if there is suspicion of standards violations. With email, this is getting quicker and easier too,
One, it is usualy sometime after a new divers Certification that they realise that soemthing was wrong with their instruction. So the questionaire gets filled out with glowing remarks unless something was so flagrant the new diver knew something was wrong right away. Kinda like when you get the survey on the new vehicle you "just" bought. Well of course everything is alright, there hasn't been enough time for something to go wrong.
Second, The instructor is usually tied to the LDS the diver will be getting gear and air from. Most would feel ackward saying something bad about their instructor if they have to face them on a regular basis, again unless it was really a bad experience.
