What CDF seems to imply are the known facts: 1) solo diver 2) with a DPV 3) who ran out of gas ~1600 feet from the exit. The event has stirred up lot of discussion about gas planning and gas planning with DPVs.

It's in a public part of CDF, not the member only accident forum
Ginnie Fatality?
Sobering straightforward cave diving fatality.

Barring any discovered comorbid disease/disorder during the autopsy & victim's medical history which might be contributing factors, an example Coroner's Report based on the above reported facts could read something like:

Cause of Death: Drowning.
Place of Death: Underwater Cave.
Manner of Death: Scuba Dive Accident, probable Misadventure involving insufficient breathing gas supply.

The public CDF discussion naturally "evolved" into a gas planning lessons learned thread for this particular case, because a general Medical Examiner's Report like the above will usually not contain any other specific relevant/revelatory non-medical precipitating factors or mechanisms leading up to the incident.
