Heart attack perhaps? Since he was able to get to the surface, was alive when he got on the boat and had only been down 15 feet (3 meters), did he perhaps overexert himself and have a coronary incident?

Just a guess.

About the only diving thing I can think of would be lung over expansion injury, although it could I suppose be something like AGE. Dunno.
