My gut says this diver was "certified" to be back where he was
I'm not a cave or rebreather diver, so trying to follow this discussion may be spotty at times for me. Glancing back over this thread, there are some details I don't see, and it may be helpful to have them here.

1.) Roughly how deep was he vertically?
2.) Roughly how far back into the cave was he?
3.) As caves go, for divers who are certified and competent to dive them, was his route considered particularly difficult? Flow, restrictions, anything good to know?
4.) Was he diving open circuit or a rebreather?
5.) Was this a solo, buddy or team dive?
 
Nothing published yet. The details of what directly caused his death are pretty inconsequential IMO, it's a reminder to us all that when a skill isn't used, it degrades and you need to take time to build it back up. He was a cowboy, his death is a disappointment but I would be hard pressed for anyone who knew him to say they are surprised.

Why hadn’t he been cave diving? Had he been diving at all? I’m getting the impression he was much longer out of the water than just the last couple of years.
 
Yeah, Andrew was a dive buddy/friend of mine. This does suck.

What matters is that he used to be able to do this dive, hadn't done this dive in a long time, and decided one day to get back into cave diving and then went hog wild. Reckless behavior was not something that he was averse to and it frankly is a miracle he hadn't offed himself earlier.

@breals could probably answer that better. My understanding is that he took a very bad hit and had him out of the water for a long time. the last time I really talked to him until last month was back in 2011 when he was on the Doria with a good friend of mine and one of the guys on that trip died. I do not know how long he was really out of the water but he was active on various forums until about 2017 or so and may have been doing some local diving at that point. Again some of his closer friends would know better

Assuming that is the overarching lesson, might details come to light that could help guide others who similarly return to cave diving after a hiatus? A specific skill that got rusty?

He was out of the water for a while, I think mainly because his job as a Dean moved him from So Cal to New York. The guy that died on the Doria was Mike LaPrade, our mutual friend and dive buddy. Andrew was his dive buddy on the Doria dive, only because I couldn't make that trip.
