Who Made/sold this Manifold? (imgs)

I am trying to verify the manufacturer/retailer of this manifold so I can get the proper rebuild kit. I was told that it was an Oxycheq manifold when I purchased it (used), but as their are no manufacturer indications other than a serial number; I would like to be sure.

Serial number is 06040005
