It's weird .....actually. When i asked our instructor whether we must have
doctor signiture. Therefore, although you suggested that i don't need it.
I must get the signiture which cost $85....... >.<
But two of my classmates filled in the form and did not get the signiture.......the instructor just asked them: Are you healthy?? and then let them go.......
