I just completed my SSI OW course, and the only time that I had that folder in my possession was when I was initialing or signing that the various training had been done. My instructor kept these folders with all of our bookwork and tests in them. As fas as buying one, I don't really know for sure, but it would make sense to keep extras of these around.

Now as far as the physical test, I had to get my doctor to sign a paper saying that he felt it was safe for me to dive because I take a prescription medication for blood pressure. We did have to do a swim, float, tred test when we went to the pool, but I feel sure that unless you are in just terrible physical condition you should pass that.

Unfortunately I'm in the mid west. In the area, there must be other SSI shops around the area. In FL, you're in the middle of diving country.
As for the physical tests (if you have no medical issues that a Dr. must sign for), there is a 300yds swim or 400yds with mask, fins & snorkel (no time) & a 10 minute tread/ survival float.

As far as the physical test, if you're talking medical, you need to get that signed by a Doctor. Your shop has the standard SSI form that the Doc signs. You need to find a Doctor or ask your shop if there is one they recommend. If it's like tstorm diver said, an actual physical test, that's a whole different story.

Actually I am having some confused.

I am very healthy and don't need to take any medication.
But our instructor still need everyone of us to have the physical examination,
to sign in a form which is about medication hisory...etc.

However, when I ask about the cost of having this examination (in my view, just fill in the form)
it costs $85!!
Is it reasonable to pay so much to have that examination?? Actually may I skip the exam??

Odd..... I would think that if you can honestly answer "no" to all the health questions on the folder, then there should be no reason to have the physical exam done. However, I'm not your instructor & don't know his motives behind requiring the physical check up. All I can guess is he is just double protecting his back side from a lawsuit. As for the cost of the physical itself, $85 isn't unreasonable, but I would expect the Dr. to do a physical, not just sign off on the paper. I had a physical (required) for my Dive Control Specialist course last year & then again this year for my instructor course. It cost me here, about $75 for each & I found out that my health insurance through my employment didn't cover sports related physicals, so I had to pay it all myself.
 
Is it possible to fill in the physical checklist form by ourselves??
 
Odd stuff here...
(1) The training record belongs to the shop, not the student. The shop must retain the record for insurance purposes. The training record should never leave the shop with a student (with a few exceptions like getting a parent's signature).
(2) The medical screening questionnaire is designed to uncover things that require a doctor's clearance to dive. Unless there's something weird in your shop's insurance policy (or they have a really paranoid lawyer) there's no need to get a doctor's clearance unless you have one of the conditions listed in the medical screening questionnaire. Here's the explanation on the questionnaire:
"The purpose of this medical questionnaire is to find out if you should be examined by your doctor before participating in recreational diver training. A positive response to a question does not necessarily disqualify you from diving. A positive response means that there is a preexisting condition that may affect your safety while diving and you must seek the advice of your physician."

Addendum: The student's record of training, complete with signoff sheets for Open Water Certification, is the SSI total dive log.

Excellent info there Rick. You are absolutely correct. Our shop tends to send the record folders home with the student to fill out the preliminary parts (name, address & such) at their leisure, when they pick up their student materials. We ask them to bring the folder back with them, along with their student materials, on the first night of class. At that point the folder doesn't leave the shop, except when the instructor must take it to the pool or open water sessions. We do have a few students lose the folders from time to time, but it is generally very rare.

The medical part you have is exactly what I've been trying to say in a round about way, I guess. Thanks for making that much clearer.
