Do you mean "lost while diving" or "lost in the swirling vortex of gear-eating darkness that is my house"?

Diving -- *&^$*^( snorkels. Two of them. I have since stopped diving with a snorkel; problem solved.
 
Let's see,

3 or 4 masks and a snorkel on one of them, all lost off a boat or smashed.
A drysuit hood, gloves, and another mask that got stolen on a beach
A weightbelt and $400 computer when my buddy flipped my kayak by tying off on the back of mine then pulling his kayak sideways.
Then we have several dive knives all of which were lost due to crappy sheath clips/locks
 
Humm lets see,

My Jet fin decided to swim away during one of my first boat ramp exits in big surge.

A pretty blue rash guard blew off the boat while drying during a SI in Sulewesi Indonesia and is probably still floating somewhere around Lembeh right now.

I've found lots of dive weights and snorkels and a Suunto computer with 43 minute BT -- luck would have it that the owner was on our boat.
 
When I passed OW I bought myself a hugh Scubapro knife to celebrate. It fell off my leg two dives later (and I did not even notice until I got out of the water)
I also dropped snips out of their webbing sheat but was able to retrieve them, I added a small bungie loop to the sheath and now they are secure.
 
I'm keeping the topic broad because obviously I didn't loose my wetsuit while diving. I set it on the roof of my car after the dive as I was loading up. That was the last time I saw it. I didn't loose my crotch strap diving. I'm just not sure where it went.
 
1. one SeaLife DC1000 camera (let go of it and it floated away)
2. SeaQuest weight pouch and 3 lbs weight
3. writing slate
4. snorkel
5. tightener on Dive Rite wreck reel
 
1. compass, I stupidly threw it across a boat in rough seas to be with the rest of my stuff and it bounced and went over the side. My computer was thrown at the same time and I am SO GLAD that did not follow the compass.
2. Spool. One and only time I put an SMB up at the surface rather than at depth, I was unclipping the spool and dropped it

One of my buddies has lost the following of my gear:
-camera :( :(
-weight pocket on my old BC

I'm doing better than some though so can't complain... recently some divers lost a full twinset and stage bottle on a boat dive and I know a guy who lost a brand new top model DUI drysuit... :shocked:
 
Many clips and small things that were creatively borrowed on the boats. A DNY shirt that was my very favorite! Same way. I hope they can wear the medium! My whole basket was dropped of the dock by a porter. I dock the should deep muck at teh marina and found everything but my Suunto..... And then a reg that some clever diver snaked and walked away with...... I have learned much in the past years. Hot pink and purple tends to stay..... hmmm.
 
I've lost nothing DURING a dive, but 'forgot' a pair of chammy dive shorts on a Coz 6-pack, so some Coz DM probably got good use out of them....a pair of nice sandals that I have no idea when during a dive trip they went missing, only realized they were missing some time later while looking for them while packing for another trip.....during dives have FOUND a few things...a towel or two...the odd fin or two....and a UK Led C4 'pistol' light doing a night dive under a pier in Bonaire, and while it was in new condition, I have no use for it (although have loaned it out to a diver or two as a save-a-dive item) and which have it 'out' for sale at a LDS right now.
