A the end of the day that's still a human factor error rectified by a pre dive check list.
I've seem more checklists ignored than followed. Sure, they can trap errors, but only if used. No one has ever asked me if I used my checklist before I splashed, even when buddy-diving.

So perhaps we should push checklists instead of dumbing down equipment? Of course doing both would be safest but at some point we gotta let the heard thin itself.

But is it a buddies job to check you and your checklist? At the end of the day aren't we all responsible for ourselves?

I like the analogy of flying and diving. When I am behind the yoke the CFI has drilled into me to use the checklist, use the checklist, use the checklist. There's like 50 things to go through to fly a plane, if I do all of them correctly then the flight should go off without a hitch. Same as diving.

You are one type of personality. There are other types.
Sure, we are responsible for ourselves....but since we have already concluded that some are irresponsible, what do you suggest? If the diver in front of you jumps off the boat without his fins, do you hand him his fins, or let him flail? Where do you draw the line?

You're 100% right. I don't always see the forest through the trees. I wear blinders sometimes.

With that in mind, I don't have any suggestions.

The problem is that diving is simpler than flying a plane and the risk of failure isn't as "in your face". Also, pilots inherently feel more responsibility to passengers than divers do to buddies, who are more or less "equals". Lack of fear breeds complacency.
 
It's either the neck o ring or burst disc.
Sure. It could be those things.

It almost certainly won't be, though. A neck o-ring isn't going to randomly explode underwater (though I have seen that once). I've seen burst discs go, of course. During filling.

But valve handles, the mechanisms under them, I've seen them go on a meagre handful of occasions.

Being able to isolate was pretty useful.
 
I read this 5 times and still have no idea what you are talking about. I guess my reading comprehension is not what it should be.

He's likely a chatbot. Someone else picked up on it, the more I read his posts, the more convinced I am that this is the most likely scenario.
