Has anyone seen one of these before? It locks on tight when the valve is opened.

Mk VI (Mk6). Internally the same as an early mk5.

When i worked in key largo we had a similar fill yoke for the tanks. Set it in place open the valve and it locks itself with the pressure.

Yes we have seen them...

I was actually an "Expert Witness " in a diving death because of this type yoke
