Sorry, but I have to disagree with almost everything Ioannos said...

As far as never having seen a reg freeflow due to cold water, my only assumption is that you have not done much cold water diving. ...

42F is my coldest so far. Have never seen a reg freeflow due to cold, down to 42F. 1000+ dives, half of them very cold.
 
Ive seen plenty of regs freeflow in cold water. Even cold water rated ones. The colder it gets the more chance of a freeze especially if someone purges it, breathes it on the surface, has it tuned too high and removes it from the mouth and many reasons.

I have to agree and a lot of the free flows are diver caused.
