Probably an older manifold. Is the problem...
Newer ones probably have a bigger hole...
The DIN nipple (this is what it’s called in the atomic service manual) has slightly larger diameter on this reg vs two other atomics I just measured. Replacing is going to be easiest. I’m headed to see if I can just replace the nipple or will I need a full DIN assembly.

The only disadvantage is that an other regulator may have the same problem....

Probably from the era that nobody had standardized din measurements...
 
Just drill it out with everything assembled resurface the face then crack the valve to blow out the swarf
