What do you carry? Which plan? Cost?

Also, the padi form asked if you are taking any prescription meds, Of coruseclike most americans I do. When we got certified I went and saw the dive doctor who cleared me to dive and told me next time just mark the form no.. as did the dive instructor. If I falsely answer this question would it void any insurance I would buy?

DAN. The PADI form has nothing to do with insurance. But don't lie to the insurance company, that could work out badly.

We do not buy "dive insurance", just like we do not buy "golf insurance".

We do buy a single travel / medical policy from blue cross that costs more than DAN and covers more than DAN.

I expect most of the divers we meet either have DAN, or no insurance. Others have different plans.

I have had DAN for 15 years ("Master" plan). Never needed it. But it's given me peace of mind, and liveaboard operators like to see it. Also DAN has a hot line for dive medical emergencies.

I'm a typical weekend-type diver who has not studied the question closely. But it you are looking for what "most divers have", it's DAN

I have DAN, the Guardian Plan.

I just joined DAN. $35/year membership. Guardian (or whatever the top level plan is called) insurance. I think it's $125/year in the US. I checked at work and my health insurance has no diving exclusions. The DAN plan I got costs as much as my renters insurance. That little extra for the top DAN plan didn't break my budget, so I figured go for it.

Like most of the others I use DAN and have their top plan.

Why not send the doctor a copy of the form and ask for a sign off that there are no issues? That way you don't have to lie or risk an insurance company using it as reason to avoid payment of a claim.

I'd recommend doing this as well. If you get a yearly physical just have them fill out the form each year and that'll keep you covered.

Like most of the others, I maintain the top level of DAN insurance. I find it cheap compared to what I spend yearly on diving.
