You might consider the Halcyon weighted bellows pocket, or one of the neoprene overshorts with pockets. The weighted pocket has room for the narrow Halcyon 6' SMB, spool, and the GPS unit/case they loaned us on the Cocos Island trip. You can also tuck a long hose under it if you're not carrying a primary light.
 
Instead of clipping to the butt d-ring, how about to the bottom of the plate with a quick link ? Should put it higher up and not hang as low as on the butt d-ring ?
 
All good ideas, thanks again for the replies! I'm going to try it stuffed tomorrow morning. I'm guessing there is no need to clip it off with it in between the backplate?
 
Transpac-right hip d ring. Backplate-lowest right hole. Jacket type bcd- d ring on the bottom right side.

It's generally good practice to clip off everything. Goofy stuff happens. I've seen a stuffed SMB become unstuffed and end up lookin like an orange tail behind by buddy.
 
When doing recreational dives I sometimes don't wear pockets especially if its rash guard and trunks weather. I found that i prefer to carry it clipped off on the back d ring. I don't wear a back pad so I guess its my only option. One thing I will say is the only time it has ever come off is during the jump in off the boat. So clip it to your d ring and then stow it once your are in the water.

But for real in absence of pockets clip it to your hip or butt dring. Pay special attention to the spool when you get in and out of the water. Prime time for it to come unclipped and dump line everywhere, which in turn makes you look like a bozo.

Big SMBs stuff em in a backplate pad.
If a spool comes unclipped from the BC for some reason it shouldn't dump the line because the clip should still hold the line to the spool. And if clipped off correctly to the spool it shouldn't come unclipped from the line and allow the line to unspool.

Try setting it up like this. I've found it to work well.

 
I usually keep a 6' DSMB stuffed into my BP pad, but try to fold it evenly to fill the pad. I also really like to replace the regular butt D-ring with butterfly hardware that gives you two rings pointing slightly to opposite sides. This makes the gear easier to reach and unclip.

When not using pockets i just clip it to my butt d-ring and pay attention to it like others said. However i love diving with my scubapro pocketed shorts that you just slip over the wetsuit, makes life easier for storing the SMB/spool, wetnotes and other misc stuff

The problem with that is it doesn't allow for the SMB and spool to be pre-assembled. Waste of time and increases your odds of dropping something.
