Failed, no. It displayed information in a non-intuitive way.
When he was looking at the screen it was displaying deco information for a gas he was breathing, but he had not confirmed the switch, so the computer continued to calculate deco info for the last confirmed gas, his backgas. His BG was a tri-mix ratio unknown to me. I got this in the post-dive post-morteum.
The big problem was that the diver did not know that his computer could display a time function other than 1:1. Also, the diver did not trust himself enough to ignore the electronic device when he had emptied a 50% O2 cylinder at 10 feet.
My observation was that an otherwise competent diver let his computer lead him astray from his planned dive.

If I get it all, the guy's buddy got out the water thinking that the other diver was only a couple of minutes behind. 20 minutes goes by and the buddy does nothing. He doesn't go to check on his mate or ask for assistance.... 20 minutes later, after eating lunch Dave notices that something isn't right and goes back in the water to see what's the matter.

Is that accurate?

How does one get a VR3 to display time in a manner different from actual time? I don't think it can be done. Sounds to me like he entered the water not knowing how to use a piece of gear he was apparently counting on. How to confirm a gas switch on a gas switching computer is important. Even if he didn't get the gas switched, if it was loaded correctly in his profile, then the computer deco times listed in the "stops" screen would have been the correct times. Talk about blindly following a computer....

the buddy, on exiting, stays topside where the down diver can be plainly seen in water with 50' vis. He converses at length with the boat captain and all on board, all are made aware of the down diver. The divers resp rate and volume are readily noted in calm water. All on the boat wonder what the heck the guy is doing for so long. 20 minutes into it, Wedivebc hits the water to check on/bring up the diver. Another tank is brought down to him and he surfaces after another ~5mins.

Repeating an earlier statement, mistakes were made. But the implications that the team had no plan or that the Buddy walked away 'whistling dixie' are incorrect.

All statements about the VR3 are hearsay since I dont own and have never used one. I was just passing on post-dive debrief material.

Yes or no: Did the buddy stay in the water while he decompressed?
Yes or no: Did the diver blindly follow his computer when he had every indication that it was providing incorrect information?

Reading it the answer would have to be yes to both.

The first (because of my DIRish bias) I find inexcusable. The second....well it should be a wakeup call to the diver, and if thats the worst that ever happens to him in his diving career ...call it a good day. I know I have gotten away with some stupid stuff, and most other people have as well.
