If I recall correctly, the weighting guidelines are in the Advanced Open Water student manual.

Can you direct me to where that guideline is published by PADI or another training agency?

It is in the Peak Performance Buoyancy training materials (with the PPB DVD or in the AOW manual). It is actually given as 10% of body weight in salt water, for a 5mm two-piece suit. For fresh water one subtracts 4 lbs for a 120 lb body weight. To compensate for an empty AL80 tank at the end of the dive, one adds 5 lbs; however, for steel tanks, "you may need to remove weight from your system." And always do a buoyancy check when using new or different equipment. It is hard to see how the boy should have had more than 10 lbs as an estimate to begin a buoyancy check, following these guidelines, which of course were not followed.

That is impossible. The intent may be for 100% of OW students to get the QA questionnaire but they don't reach 100%. More important than how many they reach is how many reply, what is that percent? It is a much more meaningful metric.
The agency has no control over how many will return the questionnaire. I imagine, that those who had a very pleasant experience don't feel a need to, while those who feel cheated would. That's just a swag on my part.

My wife and I were just the opposite. The instructor and DMs did such a good job of providing a safe and fun experience we wanted to sing their praises.

I suspected this, but since I am not a PADI instructor, I did not know for sure. I imagine that since there is no card issued, the DSDs don't get these questionnaires. That might be something PADI and the other agencies need to address.
My son did a DSD in Aruba in October. I also signed up for DSD, even though I am already certified, so that I could go along with him on the dive. We both got questionnaires/surveys emailed to us from PADI about the DSD course that we had taken in Aruba.

BTW... I completed my survey and had him complete his and we both enjoyed the dive.

I only got certified because I did an open-water DSD in Aruba years ago. My wife did a shore-dive DSD in Cozumel during one of the Invasion trips and decided to get certified after that. She did her OW while in Cozumel that same trip. I can honestly say that without doing DSD's in Open Water neither one of us would be certified today.
