I was thinking of the type 2 hit in the Baltic area. I forgot about the St. Lawrence ice/cave diving triple failure AGE incident. Ok there are 3 "DIR accidents" to consider in the past couple years. Now the OP should find 3 others as well documented and discussed in the non-DIR community (good luck, I know of 2 cave deco dive tox events in FL and one "cave" AGE in NE Canada which are well described. I don't agree with the casuality for the latter's AGE).

I'm not sure what any of this will tell you statistically, but it might illuminate how people make lemonade (or not) from other's tragedy.
