Info 1st stage failure Apeks DST

A dive buddy of mine had a about 8 dives on her new reg when the HP seat failed and delivered high pressure to the 2nd stage and the LPI. We were not underwater at the time but the 2nd stage free flowed and her wing fully inflated. Would most likely have been a highly dangerous ascent to the surface if it'd occurred under water.

I now practice removing my LPI with one hand while the other is on the dump valve. Not a skill that I'd practiced before.

XTX50 is the second stage. Does she have the DS4 or the DST (this has the rotating turret?
 
You can also get "hose hats" for BCD hoses, which are easier to disconnect (such as 1-handed, with gloves, etc).

It doesn’t make a difference what first stage it was. This is a known issue with Apeks regs and is due to the seat.

That may be, but the XTX50 is not a first stage. Will avoid confusion for others who don’t know the difference.
 
While you are correct, Apeks markets their regs based off the second stage. As for avoiding confusion, it makes zero difference as this could have happened to a DST, FST, FSR, DS4 and the list goes on……
 
Can someone explain this seat issue in a little more detail? (apologies if this is already discussed in detail elsewhere, a link or search term would suffice)
 
Explanation in a nutshell: Apeks HP seats are known to suddenly fail and cause the IP to skyrocket.

Supposedly Apeks has “fixed” the HP seat material but there are still failures happening occasionally. I know @macado has had recent failures and @kensuf has had failures before. I’m sure there are others, but those are two people that have mentioned failures to me.

The old blue service kits seem to have the issue. The rumor is the newer service kits in the white packaging do not have this issue. Apeks never officially acknowledged there was an issue and seemed to quietly fix it.

I'm using third-party Apeks HP seats and I seem to be ok but I've had ~2-3 premature HP seat failures on brand Apeks DS4s/Tek3s just out of the box with very little dives and no flooding.
