I use a BP/W, so no pockets on the BCD. But most of my dives are dry, and I use a trilam DS with tho big "tech" cargo pockets. During the dive, those pockets contain my dSMB rigged with a spool, and a fold-up snorkel. My primary light is on a soft Goodman, and my backup is clipped to my shoulder strap, GUE style. I carry one knife on my left calf, one on my waistbelt and a line cutter on my shoulder strap. And I don't need wetnotes; I have a slate on my left forearm. Whatever trash I find, typically fishing line, lures and stuff like that, is of course put into the pockets.

I still haven't figured out the best way to carry my dSMB and my backup snorkel on wet vacation dives. So far, I've managed without the backup snorkel, and my dSMB has either been clipped to a D-ring, or left on the boat.
 
I either use the pockets in my dry suit or neoprene shorts with pockets. I put a DSMB/Spool and spare mask in the right hand pocket, and sheers, my truck key and a whistle in the left pocket. I have a line cutter (trilobite) on my belt but I like to have a backup cutting tool in a second location.
 
Thigh pocket carries my DSMB rigged to a reel with trauma sheers on the side. Light is always clipped to a d-ring. Sometimes I will also use a light on a soft goodman, but I wear the pocket and carry the light on every dive.
 
Not much room in my old pockets due to 10 lb. weights under them (ie. the pockets are "tight"). On rare occasions I stuff my foldable snorkel there--if there is significant current that may change quickly. I always have my shell collecting bag there, which doubles as a fish bag should I get a flounder or two. I clip the bag cord to a D ring as I had an empty bag float out and away on me once.
 
BPW here, so I wear shorts with 2 thigh pockets.
