Back in the day, I saw one of my brass and glass shoot the face across the room. The other was plastic belonging to another diver and I just heard it sailing by my head and hit the wall. I haven't seen a failure in decades but I still face them to the ground when pressurizing.

Had a pressure gauge explode last year in the shop. It wasn't an SPG but was the liquid filled pressure checker that you use to check the pressure in a tank. It blew glass and liquid all over the walls and the ceiling and the guy checking the tank pressure. Luckily he wears glasses so he had at least a little eye protection from the debris.
 
Just looked at mine and see that the tight rubber boot would prevent any safety plug from working even if there is one, which I cannot tell because of the hardened boot.
