As others have stated, I always keep copies of a form I created, signed by my doctor, that gives me clearance to dive. I get this updated each year when I get a physical. I do have some conditions which might preclude diving without this form. I have only had one instance, in St. Marteen, that they questioned anything. This was before I started using the form. They called back to the states to contact my instructor to get authorization (wierd). Since then, I always keep the form with me and extra copies in case they want to keep one. I always make sure that my dive buddy and/or any others I am diving with are aware of any health issues.
 
The only problem I can see with carrying a standard letter from your doctor is that he/she did not sign the operators' form. With the way litigation has run amuck, some ops would say "A note from your doctor isn't good enough. He/she has to fill out this form."
One of my instructors told me I would only have to fill out these forms prior to taking a class. He was wrong. I was asked to fill out a medical history form at a resort in Bonaire in August.
