Are there any statistics available on DCS occurring:

a) Due to diver error
b) Due to computer failure
c) Due to other or unknown causes

Some of these points can be deduced from existing accident reports. From helicopter view it basically boils down the to the following:

1) Accidents occur at about 2% rates
2) of that 2% a portion can be accounted to DCS
3) of that portion most can be explained based on known risks (equipment failure, dumb stuff and ignorance of known risks)
4) A small portion of those can be attributed to esoteric stuff like tidal waves and alien abductions, PFO's and not diving DIR.
5) and a portion of those can't be explained.

I am a DAN member but have not seen published any statistics recently.

The last time I worked up a fifteen year summary (and that was about 15 years ago) it fell out like this:

1) Commercial Diving, about 1 case per 1,000 dives.
2) Recreational Diving, about 1 case per 10,000 dives.
3) Scientific Diving, about 1 case per 100,000 dives.

Things have likely shifted a bit toward more cases per dive as divers have abandoned the rather absolute no-D approach of 2 and 3 at time of the study.
