How can a person who voluntarily participates in an activity that has known health risks that costs hundreds or thousands of dollars per year possibly defend their position? By saying "because it feels good?". Might as well say the same thing about heroin.

Again, welcome to America, where people need not defend their habits, good or bad.
 
Chronic long term tobacco smoking is an addiction. Considering how deadly some of the contents of tobacco smoke, (stroke, coronary artery disease, emphysema, lung cancer, etc) There’s no other way a sane person would participate in that activity knowing those risks. IMHO.
So there are millions of insane people running around the world?

How do you propose to cure them?

You're talking about eating at MickeyDee's, right? The "contains up to 10% meat products" mince fried on emulsified "I can't believe it's not butter" parabene? Wrapped in "I can't believe it's not styrofoam" bun? Washing that down with caffeine dissolved in fructose syrup? -- If I had to choose between that and heroin, it'd be really tough choice, but apparently there's "billions served" and nobody's getting all sanctimonious about it.

There are similarities for sure, however eating is something we all do, we gotta do it in order to survive. Some people eat too much, that's why they're fat. Smoking is another issue entirely. We aren't evolved to light sticks of burning leaves and inhale them. That's a learned habit and behavior which is unnatural.

It's all bad, the difference is simply a matter of degree.

... and then you die. How could anyone in their right mind participate in activity known to end in death? Life is indefensible!
