Same as most of the above. Mine goes back to 1968 in San Diego with a cert number in the 2000s. Yes there are still some us old school divers left. Heading to Caymans for a couple week at the end of the month.
 
NASDS certified in 1967 #867 through Sea N' Ski Inc. Milwaukee, Wisconsin. Updated course in 1970 in Rhode Island. PADI Certified in 1966. Still hold active PADI certification MSDT #12. Just celebrated 45 years as PADI instructor. Received nice glass free-form trophy and write up in Under Sea Journal. Not a gold watch, but what the heck, can't put a value on 58 years of commercial and sport diving adventure.

Yes, I was NASDS-certified in Santa Monica, California at Tex's School of Diving (school 999) in July of 1972. The instructor was Gary Ray OWI 517. I was 16 years old at the time. My card still exists, but is fragile, warped, cracked, and bubbled where the microfiche window is. Parts of the plastic card have chipped and fallen away. I doubt if it will endure another dive trip with me. So I emailed SSI to get a replacement. I was told that as of February 2012 replacement of NASDS certification cards is no longer possible. I was told I had to be re-certified through an SSI dive shop or agency by either re-enrolling in a class or be evaluated by an SSI agent. I told them I still have the card (I didn't lose it) and all the information is still legible. Nothing doing, they said. Bummer.

Kevin was muy Instructor at UDT Training in Coronado. Tony Z was a teammate and a great friend. Worked for Kevin and San Diego Divers Supply for some time.

Certified by NASDS in 1972 by San Diego Divers Supply. number R7447. Instructer was Ernie Brassard #108. Received relacement card in 1976. Again applied for replacement card in late '90s. Was told records were lost in a fire at some point. Was told NASDS was no more and to apply with my NASDS info with SSI at which point I was given Open Water Diver card with new number. Back of card lists original location of certification and states replacement card for NASDS.
P.S. Found my 1976 NASDS card and still have it.

NASDS certified in 1978. The dive shop was Divers' World in Silver Spring, Maryland. Check-out dives in the quarry at Willow Springs, Pennsylvania.

My private instructor was the great Mike Hillier, OWIC 263, who later certified so many divers in Virginia Beach.

I was first certified NASDS in April 1980. All my certifications have been NASDS or SSI (merged with NASDS) except I have a nitrox card from TDI. My most current certification is SSI Dive Control Specialist Instructor in 2009.

Yeah, you waited just a hair too long to ask for a replacement. You don't really need to take a full course, you might not really need to take a course at all. If you have a log book showing 4 or 5 dives within the last year, your local SSI store may "evaluate" that as adequate. Then they can order a new card for a nominal fee. Or you could sign up for a "Scuba Skills Update" course. This course can be completed in the pool-only. Talk to your local SSI dealer, I'm sure they would find some way to accomodate you.
 
There was a question on whether NASDS instructor numbers were issued starting at 1000. In an earlier post I said I had an NASDS instructor number of 867. In 1967 I was certified NASDS instructor through Sea N' Ski in Milwaukee, WI. I took an update in 1970 and again in 1973 in Rhode Island and kept the same instructor number. Hope this clears up any question. Enclosed are two certification cards: One is mine and the other is a student who took the course from me some time later. The card are not long for this world so I carry my PADI #12 MSDT card instead. had to use a little contrast on the text. 1973 NASDS Card mine and Warrens 2.jpg
 
My name is Bob Wood Trained at Rhode Island Divers back in 1973, Larry Silva and Fred Bergman,Great instructors,Does anyone know to get replace c card;if you don't have your card number and the dive shop and NASDS no longer exist ????
