Above is a link to the new updated version of the log book inserts. I have incorporated some of the suggestions made by some of you in the previous insert thread and changed a few things I thought might make it more serviceable.

I'm open for more constructive criticism. Any new/other suggestions?

Content and format-wise, it's beautiful; I like it. I just printed it out, on both sides of a sheet, then cut it exactly into 2 halves. Then I punched 3 holes into each half. The top half is fine, but the holes impinged on the first line of the second (bottom) half of the log sheet; one hole went through the word "Site" for example. So the only minor suggestion I have (if it's doable), is to "move" the bottom half down by say a 1/4 inch. Otherwise, I'm ready to make mass printouts on both sides of the paper... pending your permission, of course. Good work, and thanks for sharing with all of us.
 
Can you make a version with the top duplicated on the sheet? That looks very much like what I'd like to have, but I have no use for the 2nd sheet.
 
The sheets have by no means been aligned for printing and punching holes just yet. I decided to leave that to last so I can get the layout finalized first. The final sheets will be nicely aligned and the punch holes set up as per standard measurements for the three ring binder. Also the final print sheets will be either normal sheets or instructor sheets; i.e. there will be two separate files, one for each type of insert, since not everyone would need the instructor sheets.

The following is 'picky' and is 'IMO' so please feel free to ignore me. (As if I needed to tell anyone that :D )

Three locations for Nitrox: Under dive type, again near viz (EANx) and the EANx % section.

Usually a check box group indicates that, in the extreme, none or all may be checked.
No 'mutual exclusivity' is possible, unlike m/ft or bar/PSI where its one or the other but never both. Based on this it might be a bit easier for migration to an automated dive log if:

[]fresh water []salt water could be replaced by Water: Fresh/Salt
[]Steel []alu could be replaced by Tanks: Steel/Alu

and so forth.
