I received many suggestions and criticisms, for which I am truly grateful.
I will review and redo my research, but I appreciate everyone who responded.
I will use the data collected so far and create a new and improved form.

I will post the results of my dissertation here soon, as well as the new version of the form.

I didn’t see the survey link, and that’s not why I’m replying. I just wanted to apologize for the unkind feedback you've received. It's disheartening to see such negativity and diatribes on this forum, as I believe healthy dialogue, even when disagreeing, can be conducted with kindness and respect. It’s unfortunate that offering grace seems to have become a lost art. Best of luck with your dissertation!

I am indeed rethinking my approach after several comments. While I am paying attention, absorbing the criticism, and trying to improve my project, there has been a lot of heavy criticism that should have come from my advisor but didn’t, and I now truly see how much I need to improve.

The advantage is that I am in the first month of my dissertation—my program started in March—and I had planned to use multiple forms, so this wouldn’t be the only one.

But I will redo everything.

But I’m glad you’re finding what’s useful in the feedback, and using it to make your project better - and glad to hear so many folks have filled it out. I did the survey yesterday.

When I did my master’s I had to have a clear question that the survey should help answer. I didn't get that impression from yours.

I’ve started working on the theoretical framework to clear my mind a bit.

But I’ll soon share the updated form here and later the results.
