I have not been to Lembeh but I have been to Dauin and it was exceptional
As an Indonesian Combo I would try and squeeze a muck diving destination with some reeefs
From a practical point of view most likely your itinerary will center around Bali as most flights leave from there or otherwise from Jakarta
Also in September or October is not really the perfect time for Raja Ampat but you could try
Bali + Komodo
Ambon + Flores
North Raja Ampat (south is windy) + Lembeh
The North Raja Ampat + Lembeh can also be done land based you don't need to be on boats all the time
 
I would definitely be interested in the the wiz slate, as a beginner I would find it a very helpful tool. Great shots by the way, after watching your video you can see how the Pygmies got their name, man they are tiny.

If you're coming from the US, the best way to head to Lembeh is on Singapore Air (or any of the code shares). You fly into Singapore and then catch a direct flight to Manado and then it's ground transport to Lembeh... arranged by the resort you are staying at.
 
From Manado you can go to Sorong with propeller plane for Raja Ampat or to Bali with a jumbo

I am not sure you can go directly to Bima or Labuan

Anyway Singapore-Manado-Bali-Singapore is doable

Many people underestimate the diving in Bali and this is wrong: first the diving is 1st class second the place is wonderful
