About the cold lips, I usually can do it if I use the corner of my lips rather than the middle: keep the mouth closed and move the DSMB inflator on the corner of the lips.
 
when on OC, I prefer oral inflate so that I transfer gas from my lungs to the DSMB and maintain constant depth. Upon release, I then inhale.

For CC with a gag strap, I'm not orally inflating kaka. Have to be fast with inflation (something I need to practice more). Though with the Kirby Morgan MOD1 with Hollis BOV Pod, I should be able to orally inflate without issue. But that's down the road.
 
Make the string tight and they don't unspool

Personal experience with open bottom SMBs…. They seem to have a remarkable talent for part filling themselves with water. If they’re a bit flaccid on the surface it seems the mouth gulps in water when they bounce up and down.

Closed bottom SMBs don’t do that, although there is still some water injected through the mouthpiece even if using an inflator hose.

Have always been nervous of inflating with an 'octopus' as it's so easy for something to snag when the bag's full. Too many stories of divers being dragged to the surface for my liking. This isn't a problem with an inflator hose as they're straight and not easy to catch something if letting go.
