Is the IP within range? (135 +/- 10 psi)
Have you set the cracking effort toward the bottom end (1.0")
Have you attached the low pressure hose directly to the air barrel, i.e. no tool or elbow in between?
Is the cutout in the flow vane pointing toward the mouthpiece? (It should)
Is it in the VIVA position? (It should be.)
Is the adjustment knob all the way out? (It should be.)
Did the balance chamber pass the bounce test?

If all of the above is OK, then you'll have to start swapping parts out one by one. You mentioned the spring-that might be a good place to start. Next the lever.

I have two G250Vs and last I tried the answers to all the above is yes. But with mouthpiece on. Cutout of the venturi vane is towards the mouthpiece, but switching it on one reg made no difference. I ordered springs and levers but it'll take some weeks.

So it looks like there are 4 such regs...
 
Before getting too carried away replacing parts, ensure the lever legs are not binding on that silly clip-on collar.

FWIW the flow vane on a G250V has a marking for orientation, which results in crescent facing mouth piece a la SP G250V service video, no matter, I did swap it around for sh.ts and giggles but it made zero difference.

Given out of around the 30-40 I "made" and these are the only two I've ever experienced this with and is the only time I've used .218 springs, logic would point to the spring.

I'll swap one out in the next few days and then we'll have an answer or at least be able to rule that out as a factor.

He's probably waiting for you to check the clearance on the lever legs and replace the spring. :-D
Uh, yes.

This has been a fascinating discussion! I would be surprised if the two springs are different.
My one question has been: are these curly foot levers?
Have never had a G250 that wouldn't freeflow without a mouthpiece.
Like I said, fascinating!
