Is this yoke insert damaged?

Please excuse my newbieness.. just got my brand new tank and the valve was like this - is the yoke insert in the the picture damaged, or is it by design, a notch to make o ring removal easier?

I thought the same thing the first time I had seen some of the inserts on my tanks.
The dent in the insert is intended to be there so the o-ring does not blow out, or so I was told.

The insert is designed with the pictured dimple to allow air to vent when removing the regulator so the o-ring doesn't blow out.
