The regulator in the article looked like a Conshelf 21 Octopus. The cracking at the metal/plastic interface was a known problem at the time and USD issued a recall on the body to replace the defective ones with new ones. The "OK" ones were a neon yellow, the "bad" ones a mustard yellow colour.
