I suspect the purpose of the sheets is to see which button you click, to assess internet user website behaviour based on user metrics, the subject matter of the website is probably irrelevant. The aim is analysis of male/female right/left handed - which buttons did you notice and use, to help assess optimal button placing on the page.

I also think the question that is actually missing though is what type of device and screen size you are viewing the pages on, with a portable device (PDA, iPad etc.) the amount of screen real estate you see, and therefore which buttons will be available to you at any one time will be different and therefore influence your choices.

I only saw one design. I clicked on one of the Book Now buttons, as requested, and answered the questions. That was the end of the survey, as far as I could tell. I did not seem to be presented with any additional designs.
