His dive physician does not seem on top of things......maybe some other, more current, dive physician?

If I knew somebody in the OP's area of the world I might be able to make a recommendation. The only guy I know is in Oman. For clarity, I think that the OP needs to take the DAN guidelines on diving with diabetes back to both his endocrinologist and his diving physician and have a conversation with both of them based on that.

For @Sidemount Serge , DAN is not just an NGO. The DAN guidelines on diving with diabetes were written by Dr. Neal Pollock, one of the foremost researchers on diving in the world; Donna Uguccioni, at the time a prominent DAN researcher; and Dr. Guy Dear, a board-certified undersea medicine physician. The workshop was co-sponsored by the Undersea and Hyperbaric Medical Society.

Aftear damaging my lungs and throat with inhalation of a liquid Sofnolime soup at 20+M. I was in the hospital and rehab for a few months.
There was no way a normal doctor could have formed an intelligent opinion about my ability to keep diving for the normal $50-100 fee.
My follow-on diving medical certification was a joint effort of several specialists:
a Professor of throat medicine for my damaged vocal cords
a Professor of cardiology for my heart
the best Pneumologist within the greater Heidelberg/Mannheim metropolitan area for my lungs
and a very good Radiologist with the most modern thin slice CRT machine in the area.

The consensus was my body was sufficiently healed that I could dive safely without restrictions.

A quick update, one more dive and I will finish the Tec 45 course.

I went to another dive doctor that gave me the all clear after extensive testing, a week later the previous doctor also gave me the all clear for the Tec course.
