Does PADI offer a "Logbook Course?"

Yes, that is called the Logbook Diver Specialty course. It is offered through joint effort with the Dork Diver group. Unfortunately, you are mandated to wear a pink snorkel, and split fin for the OW dives.
 
I haven't been diving in about 5 years, Does anybody know if there is a website that shows you how to fill out a dive log..It's been that long and sad to say I have forgot how to fill it out.....Thanks

The dive log is the least of your concerns. If you don't remember how to fill out the dive log then you probably don't remember how to dive. Take a refresher.

The log is primarily for you. The only info of vital importance is the profile / pressure group if you are diving tables. The rest is for your reference.
