I accidently bought a first class ticket once. What you get with it is you get moved to a waiting area with benches at the Playa pier and are first to board. There are some nice seats and tables on the newer ferries and you get first pick. To me it's not worth it. I prefer to sit at the back, outside if possible. The mid day ferries can get pretty full with the resort crowds going over for day trips.

The price difference won't effect people who use it a few times a year that much, the locals are the ones that will feel it the most. I haven't been going to Cozumel long, started less than 4 years ago. The bus tickets have gone up, Ferry tickets went up. When the Peso value went down the tourist bars raised their beer prices, new hotel tax. Overall it is still a very affordable but it is inching up.

It is worth it if the ride is smooth and especially if you have a group as it is quieter and has large tables to sit around. It is upstairs so if the ride is rough it will be smoother downstairs, not in first class.
Ugh. I climb a set of stairs for breakfast, but try to avoid that with my knee. Ok thanks. I'll stay below. I usually sleep anyway. A conditioned habit I've developed riding a car, plane, or ferry.

You sleep for the 45 minute ride? I would be afraid I would wake up back in Playa.

I'm glad I'm not trying to make a living running a ferry to Cozumel for 11 dollars a ticket and 6 for locals and three for seniors. That's not easy money and I can't remember seeing them close to full. They wouldn't be full if there was only one an hour.
We took the car ferry over and back this year. 500 pesos for up to 4 people on one boat and 400 on the other.

I guess they would wake me and tell me to get off if needed, but I always wake up on approach. Riding in any vehicle is just so boring, even with my daughter's scary driving, that I doze off. I try to avoid that if driving.

Including the vehicle?! Ok, only 3 or 4 crossings a day, and takes twice as long with boarding & leaving I guess.

The ferry ride can be quite entertaining.
Click link above for short video clip.
It's part of the experience.
Just remember, there is no diving allowed off the ferry.

Vehicle included. Nice ferries, nice ride. Had the vehicle full of scuba equipment. I'm not sure it takes a lot longer if you are carrying stuff. You don't have to deal with Playa congestion and on both ends when you get off you are on the road almost immediately. Actually more like 6 crossings a day.
