As Sharky and Christi have mentioned healthy should not be an issue. I have the same issue when I travel and Cozumel is one of the places where I can eat well and not have to worry about diet and weight. There is not a place I have been to that could not make something for me but some of the standard Coz fare is really enjoyable, healthy and low carb:
Cerviche
Grilled fish (personally find it much tastier and fresher than fried) - Go to Playa palancar on a Saturday after the owner has gone fishing and you can get fresh grilled whole fish that is to die for but take lots of people with you as the platter is huge !!!
Sopa De Lima but without the tortilla.

Also if you do go for tortillas make sure to get the maize ones as they are a lot healthier than the flour ones.
