Stern ties are common on the Great Lakes when conditions are calm. I’m suspecting this isn’t a best practice on the ocean?

Stern tie-in's are never a good idea. Most accidents/sinkings happen when conditions are "calm" then conditions change and they aint so calm. Current picks up, seas pick up, you take one or two waves over the stern and that is it.
