In terms of manufacturing scale anyone doing it is a small scale operation.

And the pencil is often held by just some tubing stretched over it knotted off in a hole. Any shop running a CNC router table could whip them out in any shape you like. PVC sheet is the common material of choice.

As long as you stay with a flat sheet it's easy.

You may get more tips by browsing the DIY forum, it's a common project.

Haven't got a clue. I think maybe find out where the companies who carry slates get their's made.

Go to your local Hobbie Shop where they sell model trains. Buy some thick white "styrene" sheets. Thats all there made of I make my own very simple and cheap you could probley make 10 or more for the price of 1 at the dive shop.
