Nice, trip report. Funny thing is I was at Euro dive at the same time. Must of been there togethor. I will also write a detailed trip report of my experiences when I get the time.

If you're using north coast - according to my map, the highest altitude is 36 m asl only. There's small hills around gerokgak with the top vary between 63 - 102 m - but your car will not climb this hill.
If from Bali south to Menjangan- via lake buyan (very beautiful) - you may reach the altitude 1400 m asl.

Yea, you're right. The problem is not Tulamben - Menjangan (which is coast road as you say, and therefore sea-level or thereabouts), but rather Menjangan to the South. However just ask your driver to take the West coast road and you can substitute altitude problems for traffic problems (lots of mad truck drivers heading to/from Java).
