Looks like a good way for a freediver to drown if the boat crew didn't find a knife quickly. As a scuba diver, I may have had 100# more weight so would have hit me harder, but at least I'd have an air supply.

In your opinion, is height a factor in the back roll entry. And how high is too high?

After 2m or 6ft you have to start being careful not to generate much rotational speed or you will start to complete the somersault and you may enter head first or even face first. The trick is to just let yourself drop and as your fins come over the wall/gunwhale throw your feet upwards. This will kill some of the body rotational energy. Provided you can get a straight drop going the cylinder hitting the water opens up a path and you enter the water smoothly.
Over 5m I prefer to giant stride.
 
Personally if the drop is more than a yard/meter, I prefer the "James Bond" entry! This is a forward somersault where you over-rotate and and hit the water back first. (270 degrees on your Flip-o-meter) You enter the water at about the same angle as a backward roll, but it is more more macho and impressive.
 
Back roll in doubles is disorienting and I always feel klutzy getting myself righted. I prefer a side roll, and then clip in the stages.

In that first video, it looked like the guy's stage got unclipped from his hip.

Ok, I've watched this 6 times now. This was simply a common sense issue (or lack there of). Someone's buddy should have noticed that the other had buttoned up the bottom portion of the wetsuit top around the gunnel rail. Duh Huh.
