How do those ones in the picture work?
I've never seen those since I either use a long hose/bungeed second or just a single second stage if I'm solo.
I also seldom go into the dive shop, only for air most of the time so I'm kind of out of the loop.
You loop it around a d-ring with the round end and then squeeze the mouthpiece through the square end.

Honestly a snorkel keeper will probably work as well and is cheaper.

The Dive Rite octo holders are handy too. Has a clip, but with a bungee loop so octo can be deployed quickly.

I use about 50 cents worth of surgical tubing and a couple zip ties.

As for the OOA diver taking the reg from you avoid that by not diving with people who don't plan their dives and gas supply. I was taught in my OW class with a common recreational set up to keep the octo in the "golden triangle". This is so corrupted now that the triangle seems to be from the top of the head to dangling down behind the divers butt and to open my arms and allow the OOA diver to take the reg. Once you get some experience you see how stupid this is and switch to donating the octo to the OOA diver rather than throw your arms open in helplessness and say" Oh crap! You're out of air! Hope you find something to breathe from! Good luck!" The better choice is to hand the diver an octo which shows them that you are there to help and are able to.
 
I agree with diver 85. In an out of air situation, your buddy gets your primary reg. You use the secondary which is hanging on your neck. I have mine on a strap with a quick release clip. It holds securley, but if pulled with a medium amount of force, breaks free.

My view:
If I'm out of air I will not wait around for someone to give me their octopus- I will take it.
If I see someone who signals OOA I will swim to them, detatch and then present my octo in front of me.

I know where my own octo is in the case where someone may grab for my primary.

I have a couple of zip ties on my octopus hose, near the second stage which prevents the hose from sliding while using this octo holder.

...... and speaking to a dm, who said that because i have milfix, it's not good to have double back on itself ....

If you're using the Miflex currently available in the US (there was a recall of the older ones in 2012), it can take a tighter bend than a conventional hose with no damamge whatsoever. That was actually the case with the older ones as well.
 
As I recall there is also a caution with using a bolt snap with cave line on the braided part of the hose. Seems the hose does not like that. There is actually a special holder for the hp hose. Not using it voids the warranty if you want a bolt snap on it.

I carry some of the rubber ones pictured above in my save a dive kit. If I have an instabuddy with rental gear and no holder on their rental unit, I just give them one of mine. The one time I did not do that I wound up doing a temporary air share with a newbie (first ocean dive) because her mouthpiece came off and she could not find her spare reg since it was not where it was when she was ceritified. (Captain had her put it in her pocket).

This may not be an issue of what looks best or cost or tech vs non tech as much as it should be a matter of function. Many see it different ways. As I see it the OCTO/SPARE reg Is for MY use ONLY. so I position it such that is easiest accessed by me. My choice was using a necklace such as diver 85 showed in his post #15. I admit the tech community brought this to my attention long ago, its functionality is what sold me on it.

Its functionality for me was. 1. Its just inches from my mouth at all times. 2. the hose is wrapped around the neck not dangling. 3. It does not get pulled from the chest d ring or bcd equivilant because it cant get snagged. 4. When someone pulls the hose from my mouth I do not have to deal with both WFT and where is my octo.

From a survivablility standpont it made sence. And you dont have to use a long hose to do this with the octo.

Other issues involved in my shice. 1. If all goes right you or your buddy willl never ever use the octo. So why have it hanging in the waves. 2. If used for your buddy it will have to be somewhat longer so you dont become siameese twins with your buddy as you head surface bound in panic, and it flaps in the waves and gets pulled off the vest and dangles. 3. If used for your self you can taylor fit for your use. 4. Your buddy will probably go for the reg in your mouth, then you will need to find your octo if not below you chin.
