What does this mean? Do you mean that the equipment became faulty because of diver error such as poor maintenance or lack of inspections?

How about individual accidents? Can we get reliable info on those?

I believe what they have posted is that it was the diver error, it goes further in the report that was printed. To try and break it down any further could loose it's meaning and intention.
I will look for individual investigations, if I find any, I will post them.
