This is definitly a tragic accident I spent a couple weeks diving there in December and spent some time (well lots of time) chatting with Bill R. and Fran while we were there. Fran seemed to be a very sincere individual and as I understand it Bill was just teaching her openwater as well.

But most common I would guess
1-Fuel in the tank (oil, grease)
2-Tank couldn't handle the pressure
(pressure to high, tank degrated)
I do not believe either to be the case here.
Patience is the watchword for now; when the investigation is complete we'll have a much better idea.

Unfortunately, as I have seen with other incidents, when the authorities get finished with their investigations, the results are not publicly published. And then the parties involved will not talk due to the pending litigation.
In the end, we are still left with speculation.
 
I have set up a meeting with the owner, Bill Renneker for August 7th. This will provide them some time to regroup and get the official investigation over with.

After the meeting I will provide the facts about the accident.

Please keep the speculations down until I have the provided the facts.

Sorry if my comment was leading to a conclusion, My main point was that we dont have enough information to make any educated guesses, not to even speak of conclusions
 
Condolences to her family, and wishes for a full recovery to the injured.

One question, in the Gainesville.com link news it begins "An O'Brien woman was killed ..." Then it states her name as Mrs. Slaughter.
Is there something I'm missing here?

Was this the same shop that had a compressor explosion a couple of weeks ago?
