That is a terrible tragedy... My utmost and sincere condolences to her family and friends. Such a strange accident.

Yes, a terrible tragedy ...

Please be VERY carefull when handling or mixing tanks containing oxygen... the stuff is DANGEROUS.

Regards to all effected by this terrible event.

Was it an O2 tank, nitrox, or air. I can't trust media, seems they call everything under pressure an "oxygen tank". Anyone have more info? I would guess that an O2 tank would cause more damage

Why would the tanks exploded?
 
to many reasons and to little info to give an educated guess

But most common I would guess
1-Fuel in the tank (oil, grease)
2-Tank couldn't handle the pressure
(pressure to high, tank degrated)
